About this form
We recognize that sharing a concern or experience involving behavior or interactions on campus can feel difficult. You are not required to have all the details or be certain about next steps before reaching out. This form is intended as a starting point for a conversation.
We will keep your information as private as possible. In some situations, we may need to share limited information to address safety concerns or meet university responsibilities. Wherever possible, we will discuss this with you before taking next steps.
What happens after I submit this form?
The Community Standards Case Manager will review your submission and will contact you to schedule a meeting. In that meeting, the CSCM will:
- Listen to your concerns
- Answer your questions
- Discuss possible options and next steps
Please note: submitting this form does not automatically begin a formal complaint process.
